General Requirements
1. 1. Submit an original passport, valid for at least 6 months from date of submission and have at least one completely blank page for visa stamp (amendment pages are not acceptable).
2. One 2x2 inch photo with a white background
3. One visa application form, completed and signed
4. Pre-paid self-addressed envelope if applying by mail (It is recommended that DHL,US Express Mail, FEDEX, Certified Mail or UPS be used).
Additional Requirements for Non US Citizens
1. Evidence of your immigration status in the USA (e.g. copy of green card or
valid visa for the US)
Additional Requirements for Business
1. A business letter from the sponsoring company in the US. The letter should
include the dates of travel, purpose of visit and a statement of financial
responsibility for the applicant.
