General Requirements 1. A passport valid for a minimum of six months beyond date of departure
2. One visa application form completed and signed
3. Two 2x2 inch photos with a white background
4. A photocopy of your roundtrip flight itinerary
5. Pre-paid self-addressed envelope if applying by mail (It is recommended that DHL,US Express Mail, FEDEX, Certified Mail or UPS be used).
Additional Requirements for Non US Citizens
1. Evidence of your immigration status in the USA (e.g. copy of green card or
valid visa for the US)
Additional Requirements for Business
1. A business letter from the sponsoring company in the US. The letter should
include the dates of travel, purpose of visit and a statement of financial
responsibility for the applicant.
