General Requirements 1. Submit an original passport, valid for at least 6 months from date of submission and have at least one completely blank page for visa stamp (amendment pages are not acceptable).

3. Two 2x2 inch photos with a white background
4. A copy of your driver's license or utility bill to verify your state of residence
Additional Requirements for Non US Citizens 1. Evidence of your immigration status in the USA (e.g. copy of green card or
valid visa for the US)
Additional Requirements for Business 1. A business letter from the sponsoring company in the US. The letter should
include the dates of travel, duration of visa requested (ie. six months, one yeear or ten years) purpose of visit and a statement of financial
responsibility for the applicant.
