Passport Renewal Requirements
You are eligible for a passport renewal if all of the following statements are true.
-You can present your most recent passport.
-You were over 16 years old when your last passport was issued.
-Your previous passport was issued less than 15 years ago.
-Your most recent passport is not badly damaged.
-Your name has changed and you can present an original name change decree or marriage certificate.

Submit the following:
1. A completed DS-82 application form
2. Current US Passport that was issued no more than fifteen years ago
3. Two 2X2 inch passport type photos with a white background
4. A check or money order to: "Passport Services".

Standard Government Fees: (Allow upto 14 weeks for processing)
Adults (16 and Older) - $75

Expedited Government Fees: (Allow up to 3 weeks for processing)
Adults (16 and Older) - $135
