US Passport Information

New Passport
If any of the following statements are true, you should apply for a "new passport".
- You have never applied for a passport.
- You were under 16 years old when your previous passport was issued.
- Your passport has been lost or stolen.
- Your previous passport was issued more than 15 years ago.


To apply for a NEW PASSPORT, collect the following items:
1. Completed and printed (Do not sign application.) DS-11 Application Form
2. Proof of US citizenship
(Original Birth Certificate, Naturalization Certificate or Previous US Passport)
3. Proof of Identity (a government issued photo ID)
4. Two passport type photos 2x2 inches with a white background
5. "New Passport" applications must be witnessed by a United States Goverment official.

("New Passport" applicants must visit a US Passport Agency in person, or a local US Passport Acceptance Facilily to have the DS- 11 application witnessed. Acceptance facilities are usually a nearby post office or court house. You will be charged an additional $25 fee to have your application witnessed at an acceptance facility.)


Additional Requirements for Minors:
1. Minors under sixteen should be accompanied to either a passport agency or acceptance facility by both parents that are noted on their birth certificate.
(If one parent cannot be present, a copy of form DS-3053 should be signed by the absent parent and notarized.)

*Minors between 14 and 16 may be issued a passport with one parent's permission. This decision is at the descretion of the acceptance agent.
*Minors under 14
must submit a birth certificate as proof of citizenship.


Additional Requirements for Replacing a Lost/Stolen Passport:
1. One DS-64 form, completed and signed
(If you have lost more than one passport in the past, this form should be notarized.)


Standard Government Fees: (Allow upto 14 weeks for processing)
Adults (16 and Older) - $75
Children (16 and Under) - $60
Expedited Government Fees: (Allow up to 3 weeks for processing)
Adults
(16 and Older) - $135
Children (16 and Under - $120

(2 weeks - 14 weeks processing time)
